A Friday night parking crash has left one woman dead, according to Hoodline, after the victim neglected to put her transmission into park when parking and was run over by the vehicle.

Bay City News reports that the victim was 52-year-old Daisy Pascual of Daly City, who’d parked her car on the 800 block of Waller Street shortly after 9 p.m. on Friday night.

But according to police, her car’s transmission had not been properly placed in park on the street’s incline, and likely the emergency brake was not engaged either. When she got out of the car to check something near the rear of her vehicle, the car rolled over and caused life-threatening injuries.

Pascual was pronounced dead shortly after arriving at the hospital Friday night.

Coincidentally and unfortunately, in the same week in which she was publicly shamed for texting while driving by Stanley Roberts, Oakland mayor Jean Quan had a car accident on Sunday. Early reports suggest that Quan may have been at fault, possibly after running a red light, however Quan so far denies that — KTVU is also really eager to suggest she was on the phone at the time, but Quan says she is submitting phone records to prove she wasn't.

The accident happened at 26th and Market Streets in West Oakland around 7 p.m. Sunday, and Quan issued the following statement after it happened (which she later repeated via Twitter).

(Sunday) afternoon I was driving between two community events in West Oakland when another vehicle struck my left-rear tire. I immediately pulled over and checked to make sure no one was hurt: it appeared everyone was all right and their well-being was my first priority. Police are investigating what led to the collision and I'm going to let them do their job and report out the details. I will also be submitting my phone records investigators, which will confirm that I was not on my phone. I'm grateful that no one has been reported injured, but if anybody was hurt I hope they get the care they need right away. Please stay safe out there and look out for one another.

A witness tells the Oakland Tribune that she saw Quan run the red light, and claims she was "looking down" at the time, possibly at her phone, and not looking where she was going.

The other people involved in the accident, a woman and a 14-year-old boy in the car that Quan collided with, were apparently uninjured but both went to the hospital with "some pain."

It would be really prescient of Stanley Roberts to have called out this problem with Quan's driving just days before this unfortunate accident, if it does indeed turn out to be true that she was on her phone.

Glad everyone's (mostly) alright!

St. Helena-based chef Cindy Pawlcyn was among those injured in a head-on collision on highway 12/121 west of Napa on Sunday afternoon. The crash also took the life of a 4-year-old boy.

As the Napa Valley Register reports, the boy and his mother were passengers in a Chevy SUV whose driver, 23-year-old Flavio Castellanos of Napa, swerved into on-coming traffic on the two-lane road.

Pawlcyn's husband, John Watanabe, was driving his Lexus RX 450 and swerved to avoid the SUV, but both ended up colliding "almost front license plate to front license plate," as a CHP officer reported. "It was about as head-on as you can get," he said, and you can see that from KTVU's footage from the scene.

The mother of the boy was severely injured and airlifted to Santa Rosa, while Pawlcyn and her husband suffered minor to moderate injuries and were taken to Queen of the Valley Medical Center. The boy, who was in a carseat at the time of the crash, was airlifted to the same medical center, but later succumbed to injuries.

Pawlcyn first made a name for herself as opening chef at Fog City Diner in the 1980s. She went on to open Mustard's Grill in St. Helena, as well as Cindy's Backstreet Kitchen.

That block probably could use the bath, anyway.

The driver of the sedan was arrested on suspicion of DUI, but apparently no one was injured in the crash, as Bay City News reports.

As the story goes, the sedan hit the taxi, sending the taxi into the hydrant. The accident happened outside 691 O'Farrell street, and water from the hydrant was shooting 60 feet in the air, as you can see here in this Vine from a neighbor.

The incident happened at Seventh and Folsom Streets. According to reports, the cyclist was going the wrong way on Folsom, a oneway street.

KTVU notes:

SFPD Sergeant John Bragagnolo said the bicyclist was going the wrong way on Folsom Street and rode right into the intersection at 7th Street.

"The light was green for north bound traffic going up 7th street. She would be unable to determine what her light was because the light's don't show that way," he told KTVU.

In addition to suffering a broken bike, the victim sustained "possible broken bones and a concussion."

Not a good week for those of us traversing the city sans automobile. Today, over at Geary and Arguello in the Richmond, a driver smacked a cyclist a little before 11:30 am. "The bicyclist was conscious and alert following the collision," reports SF Appeal. "No other information about the collision was immediately available."

Today's incident, though harrowing in its own right, wasn't as tragic as two separate car-related accidents that happened this past week. On New Year's Eve, a family was struck by a car in the Tenderloin. Six-year-old Sophia Liu died as a result of her injuries. Earlier that day in San Francisco's Crocker-Amazon neighborhood, 86-year-old Zhen Guang Ng died after getting struck by a pickup truck near the intersection of Naples and Rolph Streets.

The view from outside SFist headquarters in SoMa is a pitiful one. Traffic backed up as far as the eye can see. And with that, we regret to inform you that at least three EB lanes are closed due to an accident. If you're planning on heading over to Oakland using the Bay Bridge, don't. At least not this morning.

A fatal collision between a motorcycle and a vehicle Monday night at the intersection of Oak and Steiner left the motorcyclist dead from head trauma. Today, Haighteration brings word that the victim was Billy Beckwith, a carpenter and co-host on HGTV's Curb Appeal.

The accident occurred at around 8:30 p.m. Monday night. Beckwith was transported to San Francisco General Hospital, where he died of his injuries. According to eyewitnesses, the driver of the car was turning left from Steiner onto Oak when the motorcycle came up Steiner headed the opposite direction. The motorcycle struck the car as it was turning, flinging Beckwith onto the street.

Beckwith was a 38-year-old San Francisco resident whose company, BB Design Build, provided construction services around the Bay Area. Curb Appeal has more on Beckwith's background:

Born on a 60-acre vegetable farm in midcoast Maine, Bill was taught by his father to grow food, but Bill found himself drawn to working on the old structures on the property and building things in the farm's workshop. Bill later attended the University of Montana, where he studied English and martial arts. During summer breaks he worked in the mountains of Wyoming restoring the cabins of a historic fly-fishing ranch.

As evidenced by his Facebook page, Beckwith was an avid motorcycle enthusiast, traveller and outdoorsman. A memorial page has already been set up for Beckwith with photos provided by family and friends.

Robinson's Buick had collided with the center median wall and for unknown reasons he got out of the car and was trying to cross the freeway from the number one lane to the right shoulder when he was hit by several vehicles, the CHP said.

He was found in the number four lane and pronounced dead at the scene.

CHP Officer Daniel Hill said two of the drivers who hit Robinson remained at the scene and gave statements to officers but others continued on.

Two BART workers were killed after being struck by a moving BART train Saturday afternoon. The victims, whose names have not yet been released, were conducting inspections on a stretch of track north of the Walnut Creek BART station. Although BART workers were on strike Saturday, some BART trains were being moved around the system for maintenance.

The accident occurred at around 2pm this afternoon, when an eastbound four-car train struck the pair near Jones Road and Pimlico Drive. According to the transit agency, the train “was on a routine maintenance run with an experienced operator at the controls. At the time of the accident the train was being run in automatic mode under computer control.” However, a BART radio transmission indicates that the driver was being directed over the radio. "Keep coming, keep coming, keep coming," repeats the female voice, followed by "Slow down. Stop! Stop! Stop!"

According to BART, the victims had "extensive experience" around moving trains. However, according to safety policy one employee is supposed to watch for oncoming trains while the other conducts the track inspection.

One of the workers was a member of American Federation of State, County and Municipal Employees Local 3993, a union involved in the strike. Worker safety has been one of the issues raised by the striking unions. The Amalgamated Transit Union Local 1555 has stated that it will not picket tomorrow “out of respect for the families involved.”

First we got a new BART strike, then a cyclist was killed, this time by a Muni bus. Now the day seems downright cursed, with another accident this afternoon in Duboce Triangle that reinforced the rule of bad things coming in groups of three.

Haighteration brings word of the accident, which occurred around midday when a Rogue Moving & Storage truck and a minivan came into contact at 15th and Castro, somehow flipping the truck on its side. Two minivan passengers were sent to the hospital and one traffic light was shattered, but the truck's driver managed to emerge unscathed.

How the truck flipped over is unclear, but one thing's for sure: whether you're a truck or bus driver, a cyclist or a pedestrian, please be extra careful out there today.

As we learned earlier, the scene was chaotic and confusing, and Ye may have been run over more than once by multiple fire rigs as she became obscured by fire-retardant foam. But the investigation appears to point to one firefighter in particular, and the findings have been handed over to prosecutors who will decide within two weeks to pursue charges.

ABC7's Dan Noyes previously pointed the finger at 49-year-old veteran firefighter Elyse Duckett, who arrived late on the scene because she had been out of the firehouse getting food for the crew when the crash happened and the first calls came in. Her rig, as Noyes reported, was not equipped with the heat-detection technology that's used to detect people in the truck's path in visually impaired situations.

Ye's body was spotted in a fetal position by several firefighters who concluded that she was dead. The coroner later concluded she had died from crushing injuries to her head and pelvis from a vehicle.

The final results of the investigation have not been made public.
